Mental health hospital failed to stop suicide — A report indicates that a patient died by suicide at a Portland mental health hospital two months after investigators told the facility it was failing to protect patients. The state Health Authority inspected the Unity Center for Behavioral Health in July, finding many of the same deficiencies identified in an initial March inspection. According to the report, a patient admitted to the facility in July was assessed as being at high risk of suicide. Staff recommended she be checked every 15 minutes and her bathroom door should stay locked. The patient was later found dead in the bathroom.
