On Dec. 20, 2007, Bend resident Robert Gaines said he took a hard fall on the icy parking lot outside Costco Wholesale Corp., resulting in three stitches to the head and a $1,600 medical bill from St. Charles Bend.
To his surprise, he said a representative from the Issaquah, Wash.-based company....
